What does it mean?

No other character but Kyon calls Suzumiya Haruhi "Haruhi", not to mention he refers to everyone else by their family, not their given name. I've read that it can either be a sign of intimacy or disrespect and I'm guessing it's the latter. Correct?

Prior to the Shinjin alternate reality event, Kyon called Haruhi: Suzumiya-san. But after that event, I guess you could consider that those two now share a more intimate relationship. Kyon and Haruhi are good friends despite what his narration says. Read through his text, on the surface Kyon doesn't like Haruhi that much, same with Haruhi. But underlining his text occasionally are hints of liking her or a special relationship.

Kyon likes Haruhi, it's just that he doesn't show it a lot. However, if they ended up together in a open relationship, it would be a sign of the books coming to a close. Best not to see it openly soon...

Personally I think it's a little bit of both. In regards to intimacy, he's certainly familiar with her. Despite how ever annoyed he may get by Haruhi, they always appear to talk in the morning and during club hours. The fact that they tend to talk about casual random goings-on suggests a certain degree of familiarity to me.

But then again, he's pretty familiar with the rest of the SOS Brigade and doesn't choose to call them by their given name. Which is where the 'disrespect' comes in. I can only imagine that something along the lines of "Politeness and formality is wasted on that girl" once ran through his mind. :p

Mikuru even asked him to call her "Mikuru-chan" but he rejected the offer and continues to call her "Asahina-san".

I've always kind of wondered why Kyon does that actually. In the novel he has a typical "Man, she's cute!" reaction to her request, but carries on calling her Asahina-san anyway, and in the anime he has a sort of "......." expression.

Maybe he just feels uncomfortable with it because she's his sempai? Or maybe he's worried that he'll get beat up if any guys hear him calling her so informally. :P
