Instead of using [...] to give the original Kanji reading, why don't you use furigana? Although I don't remember it off the top of my head, there is wiki-code for that. We used it for Hakomari (Vol 3 and/or 4), and I remember seeing it somewhere else as well... And wouldn't it look way cooler than some term in brackets lagging behind?

Looks good. Although there is some interference with the index for the TL note in the prologue... And to be honest, blue/purple poison+red black magic is a bit much, especially with both of them in boldface. How about adding the index to the furigana and removing the link-color (keeping the furigana red in spite of the link), or is that impossible?
Now, I'm not telling you to get rid of the red or the boldface, because when there is no link involved, it's fine. Or rather, any other way would probably seem odd or lacking.
P.S.: You might want to do something about the positioning/size in general. If not in the first line of a longer sentence, it interferes with the line directly above it by one or 2 pixels, at least for me.

The template's a great idea! I wrote another version myself, though, which might be a little more comfortable to use (chiefly because it automatically gets centered. Ah, but it doesn't work on IE6):
3rd parameter -> font-size
[margin ] -> size of the gap between text and furigana.
[spacing ] -> Letter spacing
[color ] -> color
[fontWeight] -> thinner/normal/bold/bolder/100-900
[fontStyle ] -> italic/oblique/normal
Though, it would be much easier if we could just use the <ruby>-tag.
