This comes as a shock. And she's not even that old.
One of my first animes that I seriously followed was Hikaru no Go, that was when I first noticed her, though I'm aware she first became well known due to her role in Revolutionary Girl Utena.
Bleach-tards would know her as Soi-Fon, while another ongoing role she was supposed to have continued was Fuyuki in Keroro Gunso. Both roles have since been filled in by Kuwashima Houko for some time now while she took her extended hospitalized leave, in the hope she would return. But it was not to be.
Her most memorable role for me though, was as the tragic heroine Kamio Misuzu in AIR, especially in the tear-inducing finale. Little would we have realized reality would mirror fiction, and like Misuzu-chin, Kawakami-san would also reach her "goal".

Rest In Peace.
