Hello. I left Ryuuji's own thoughts unquoted and unitalicized for a reason - please see the translation notes.

The narration is commonly in 3rd person, but it synchronizes with Ryuuji's own thoughts and feelings. Adding a distinct marker is going to ruin the feel of the original language, that's my own feelings on the matter. The novel isn't written from an objective point of view.

Second, the use of italics for the other stuff is meant to signify thoughts involved in actions or stuff in the background that aren't being clearly paid attention to by the narrator.

Third, the dashes are there to prevent overuse of commas, because frankly, it gets confusing.
