Hello. I left the thoughts unquoted and unitalicized for a reason - see the translation notes. The narration is commonly in 3rd person, but it synchronizes with Ryuuji's own thoughts and feelings. Adding a distinct marker is going to ruin the feel, or so I thought. The novel isn't written from an objective point of view.
Second, the use of italics for the other stuff is meant to signify thoughts involved in actions or stuff in the background that aren't being clearly paid attention to by the narrator.
Third, the dashes are there to prevent overuse of commas, because frankly, it gets confusing.
